Hospital bedside services

Compassionate service, at the bedside.

Mobile notary appointments for patients and families in hospitals, hospice settings, and care facilities.

ADVANCE CARE
NOTARIES

Compassionate. Professional.
Convenient. We come to you.

Important documents during a care stay

We can assist with notarization of client-provided healthcare and financial powers of attorney, advance care documents, and other paperwork that needs a notarial act.

Coordinate the visit

Share the facility name, location, document type, number of signers, and witness needs. Please arrange visitor access and an appropriate appointment time with the facility.

Careful, patient-centered appointments

The signer needs to be able to communicate and participate voluntarily in the signing. Tell us about any communication or accessibility needs before we visit.

For care teams

Case management and social services teams can call or email to discuss referrals and coordination. Please avoid sending patient records or sensitive health information through ordinary email.

Medical & healthcare documents

Compassionate service for important healthcare signings.

We visit hospitals, nursing homes, assisted living communities, and hospice settings. Families and care teams can arrange appointments for client-provided documents.

Hospital bedside appointments

We have completed bedside notary appointments at Advocate Christ Medical Center in Oak Lawn, Illinois.

Request a visit to your hospital or care facility within our service areas. Share the facility name and location so we can confirm coverage and coordinate visitor access.

Advance Care Notaries is an independent notary service. Hospital names identify appointment locations and do not imply affiliation or endorsement.

Transfer-on-Death document notarization

We notarize client-provided Transfer-on-Death documents for real estate and can coordinate witnesses for the appointment. In Illinois, these documents are called Transfer on Death Instruments (TODIs).

Families and hospital care teams can contact us to arrange a signing at home, at the bedside, or in another setting within our service areas. We confirm document, signer, and witness needs before scheduling.

Consult an attorney about preparing the document, whether it suits your situation, and recording requirements. Requirements and availability vary by state.
